Reduced Fees

If you cannot afford my fee, please let me know as I have a limited number of sliding scale openings.

Court Appearances

Court appearance and deposition fees are $400.00 per hour with a four hour minimum not including transportation to and from court house or offices. Fees must be settled before appearance and a two week advance notice is required so that I may adjust my schedule and not inconvenience our patients. Additional travel fees may be applied.

An individual session is approximately 45-60 minutes.

Cancellation Policy

I understand that situations arise in which you must cancel your appointment. It is therefore requested that if you must cancel your appointment, you provide at lease 24 hour notice. This will enable another person that is waiting for an appointment to be scheduled in the appointment slot. With cancellations made less than 24 hours notice, I am unable to offer that slot to other people. Office appointments which are cancelled with less than 24 hour notification may be subject to a $180 cancellation fee.

Payment

Cash, Credit Card or check accepted for payment at the time of service.

Returned Checks

For returned checks, both paper and electronic , a $50 NSF charge will be assessed in addition to the amount of the check.
